Juvenile Drug Testing
All youths who are on Diversion or probation are required to submit to drug screens as
directed by their Court Services Officer (CSO). If you have questions regarding drug testing, please contact your supervising officer,
or the Court Services Office at 913-715-7400.
Drug testing is conducted by Consolidated Medical Services, LLC.
Colorcode hotline phone numbers are 816-822-4748 and 816-822-4656. Here is
their information including a map, directions and phone number for their
Olathe and Shawnee
locations.
IMPORTANT NOTES:
- The forms listed above are in Adobe Acrobat 6.0 (.pdf)
format and may not work with previous versions. The current version of Acrobat Reader is
available free from Adobe's
website.
- You may download the forms to your computer by clicking your right mouse button on the
